Registration of Seafarer’s and Issuance of Seafarer’s Registration Certificate (SRC)

Documentary Requirements

Office                   :             Seafarer’s Registration Unit, Mezzanine Floor
Client/Customer:            Seabased Workers/Seafarers/Agency Representatives
Fees                      :             Php 50.00 for SRC
                                            Php 150.00 for lost SRC

PROCEDURES:

 Step 1:  Get application form from the Security Guard

Step 2:  Fill out forms and submit complete documents at the designated counters for evaluation

Step 3:  Completed documents shall be returned to the applicant for payment at cashier (Incomplete documents shall be returned with instructions to comply with the lacking requirements)

Step 4:  Proceed to 5th floor for payment of the required fees and issuance of the official receipt

Step 5:  Resubmit complete documents at the designated counter for encoding

Step 6:  Proceed to the waiting area and wait for the release of SRC (Morning 11AM-12NN and Afternoon 3PM-5PM)

Step 7:  Present the official receipt to the releasing counter for the release of SRC

 Yellow Fever Vaccination and Others

Yellow FeverGiven to travelers going to Africa, South America. An International Certificate of Vaccination (ICV) shall be issued after administration of the vaccination. A waiver certificate is issued to travelers who are allergic to Yellow Fever vaccine. Cholera vaccinationPer WHO mandate, cholera vaccination is no longer required for international travel.
This is given to travelers upon request by the individual concerned or by the placement or travel agency. Oral Cholera (Dukoral) is given in two (2) doses at one – week interval. The vaccine is available in certain drugstores outside the BOQ office.
